Key Responsibilities:
- Regularly update and maintain a detailed and accurate patient database, ensuring all relevant medical, financial, and support information is recorded.
- Monitor patient information for accuracy, ensuring data is kept current and compliant with organizational and legal requirements (e.g., data protection regulations).
- Generate reports and monthly statistics for internal use, tracking patient progress and service usage.
- Conduct thorough financial assessments of patients and families to evaluate their eligibility for financial aid, support, and other assistance programs.
- Review financial aid applications and ensure all documentation is complete and accurate before funds are disbursed.
- Monitor ongoing financial aid cases, ensuring the timely review of patient needs and resources available.
- Refer and register eligible patients who are located in other wards or facilities to ensure they receive the necessary services and support.
- Liaise with medical departments and teams to ensure smooth patient transitions and comprehensive care across different wards or locations.
- Introduce patients and caregivers to SCCS services and register patients
- Connect patients and their families to additional support services, such as social workers, counselling services, and welfare agencies, to ensure they receive comprehensive care.
- Collaborate with external welfare agencies, NGOs, and government programs to secure necessary resources and support for patients.
- Provide patients with information on available community resources and assist with the application process for welfare and other support programs.
- To assist patients/ caregivers with the opening of bank accounts as and when required.
- To plan and conduct support groups for caregivers and youth based on the modules designed by SCCS.
- Provide information and resources relevant to participants’ needs and liaise with counsellors if participants would benefit from additional counselling sessions
6. Logistics coordination
Coordinate logistics arrangements, including transportation for discharged patients to the halfway home
Qualification:
- Degree or Diploma in Social Work, Counselling, Healthcare Management, Public Health, Psychology, or a related field.
- Alternative qualifications: A background in Administration with 2-3 years’ experience in healthcare or patient services can be considered.
Work Experience:
- 3-5 years of experience in support services, healthcare, social services or non-profit sectors, preferably in roles involving patient care, coordination, or team management.
- Experience in case management, patient support services, or financial aid assessments is highly desirable.
- Previous experience in managing databases and handling patient records is a strong advantage.
- Experience coordinating with social welfare agencies, NGOs, or healthcare support services to provide comprehensive care to individuals.
