Job Description

This position is dedicated to providing comprehensive case management, coordinating program activities, overseeing volunteer initiatives, and managing educational support for residents. The position will work collaboratively with the management, care staff, families, and external stakeholders to create a nurturing and empowering environment for each individual under our care.

  • Conduct thorough investigations and assessments of new referrals, which may involve home visits.
  • Periodically assess and develop intervention strategies to formulate personalized Individual Care Plans (ICP) for each resident, ensuring their holistic needs are met.
  • Engage in individual casework, including counseling, family, and group work.
  • Collaborate closely with management, care, and support staff, including Corps Officer(s), in conducting periodic reviews of residents' ICPs.
  • Prepare and disseminate all required reports for Residents' Review Board meetings in a timely manner.
  • Participate as a member of the Residents' Review Board of the Home.

  • Qualifications
    • Bachelor's degree in social work, or a related field.
    • Proven experience in similar role is an added advantage.
    • Strong understanding and ability to work collaboratively with diverse groups.
