-Conducting initial and ongoing assessments of patients' health conditions. -Developing and updating individualized care plans based on assessments. -Monitoring vital signs and overall health status regularly. -Administering medications and treatments as prescribed by physicians. -Managing wound care, injections, IV therapy, and other medical procedures. -Assisting with medical equipment and ensuring its proper use and maintenance. -Helping patients with activities of daily living (ADLs) such as bathing, dressing, grooming, and feeding. -Assisting with mobility, including transferring patients from bed to wheelchair and aiding with walking. -Ensuring patients' comfort and safety within their home environment. -Educating patients and their families about managing health conditions and following care plans. -Providing guidance on medication adherence, diet, and lifestyle changes. -Offering emotional support and counseling to patients and families dealing with illness or disability. -Collaborating with physicians, therapists, and other healthcare professionals to ensure comprehensive care. -Coordinating referrals to other healthcare providers or community resources as needed. -Communicating regularly with the healthcare team to update them on patients' progress and any changes in condition. -Maintaining accurate and detailed patient records, including care plans, progress notes, and any changes in health status. -Reporting any significant changes in patients' conditions to physicians and other healthcare providers. -Ensuring all documentation complies with regulatory and organizational standards. -Responding to medical emergencies and providing immediate care as needed. -Stabilizing patients and coordinating transport to medical facilities if necessary. -Teaching family members how to perform certain care tasks and use medical equipment. -Providing respite care to give primary caregivers a break. -Offering resources and support to help families cope with caregiving responsibilities. Other Requirements: - Willing to work during weekend & on standby if need to replace any career. - Have own transport will be added advantage.
