Position: Customer Service (Mandarin – Cantonese)

Responsibilities:
- Conduct initial phone assessments to understand clients' needs across medical, social, emotional, and financial aspects.
- Develop personalized care plans in collaboration with clients, their families, and healthcare professionals, setting clear goals and outlining tailored interventions.
- Arrange and coordinate healthcare services, including scheduling appointments, transportation, and facilitating communication among healthcare providers to ensure seamless care.
- Regularly track clients’ progress and adjust care plans as needed, assessing the effectiveness of interventions and recommending improvements.
- Maintain detailed records of client assessments, care plans, and interactions, ensuring compliance with regulations and documentation standards.
- Provide clients and their families with relevant information, guidance, and emotional support to navigate the healthcare system and manage health conditions.
- Connect clients with additional healthcare providers, community resources, and support services to address their comprehensive needs.
- Contribute to initiatives that enhance care delivery and optimize client outcomes.
- Communicate effectively with clients, families, healthcare professionals, and stakeholders to foster collaboration and continuity of care.
Qualifications:
- Diploma or Bachelor's Degree - (Degree required for non-Malaysian candidates)
- Minimum of 3 years of relevant experience
- Fluent in Mandarin and Cantonese
- Willingness to work during all Malaysian public holidays
- Available from 7 AM to 4 PM, based in Kuala Lumpur
