RESPONSIBILITIES To provide quality basic care in accordance with hospital policies and procedures under the direction of a professional Registered Nurse: 1. Assist in patient’s admission • Prepare patient room and bed for new admission. • Take height and weight of patient • Orientate new patients to their assigned bed and facilities of ward / unit. • Document correctly on Admission Checklist • Feedback to Registered Nurses of any requests or concerns from patients. 2. Assist in monitoring patients • Monitor and record patient’s vital signs e.g. temperature, pulse, respiration, blood pressure, oxygen saturation and pain level • Document correctly on Observation Chart. Report the outcomes of observation to Registered Nurses. • Perform Haemoglobin Glucose Test (HGT) for patients. Record and report HGT result to Registered Nurses. Perform glucometer quality control test. • Assist Registered Nurse in performing 12-Lead ECG recording. 3. Assist patients in performing activities of daily livings (ADLs) according to patients’ needs • Nutritional – assist patients in menu selection and in feeding patients during mealtime. Assist in nasogastric tube feeding after RN confirmed the tube placement. Assist patients with intravenous therapy and removal of IV cannula. • Elimination – giving and removing of bedpan / urinal / commode, assist in toilet rounds and change of diapers. Assist in applying of Uros heath/condom catheter if needed. Assist in urine and stool specimen collection. • Hygiene care – assist patients with shower or bed sponging, provide skin, hair, nails, oral and perineal care. • Activity and physical function – assess patients for pressure injury and fall risk, assist patient in turning and change of position, assist patient in and out of bed, to chair / wheelchair and to toilet. 4. Attend to patients’ call bell and determine their needs. 5. Make beds for occupied / un-occupied and post-surgery beds. 6. Act as chaperone whenever need arises. 7. Transport patients to and from other departments, for diagnostic purposes or discharge, as requested by Registered Nurses. 8. Deliver documents or specimens to other departments as required. 9. Assist Registered Nurses when they are performing nursing procedures as required e.g. wound dressing, catheterization, venepuncture, cannulation, administration of nebulizer and etc. 10. Assist Registered Nurses in performing last office. 11. Taking inventory stock level and helping with basic maintenance of ward items. 12. Perform housekeeping duties such as maintaining cleanliness of items used by patients such as table tops, cabinets, crockery, urinal and bedpans. DOCUMENTATION & PROFESSIONALISM 1. Perform accurate documentation of nursing records at all times for: • Admission Checklist • Discharge Checklist • Clinical Observation Chart • Fluid Balance Chart • Modified Morse Fall Scale Assessment Chart • Pain Assessment Chart • Pre-operative Checklist • HGT Chart • any other documents as deem suitable 2. Communicate any abnormalities detected and report to Registered Nurses in a timely manner. 3. Provide care and perform job according to nursing code of ethics. 4. Report unsafe practices, professional incompetence, and professional misconduct through appropriate channels. 5. Ensure the dignity and rights of patients are not infringed upon by other members of the hospital staff and to always maintain confidentiality. 6. Ensure continuous enhancement of knowledge by attending training sessions relevant to job performance. 7. Establish and maintain good and effective interpersonal relationship with patients and their relatives as well as other inter-disciplinary team members. 8. Communicate effectively and professionally with patients, patients’ relative, nursing team and other healthcare professionals. OTHERS 1. Perform other related duties as assigned from time to time by Registered Nurses, Assistant Nurse Manager / Nurse Manager and/or Nursing Director. PRIVILEGING 1. The Patient Care Assistant will be privileged (3 yearly) on: • Obtain Height and Weight Measurement • Obtain Body Temperature Using Tympanic Thermometer • Obtaining BP Using Electronic BP Machine • Checking HGT for Patient • Perform Glucometer Quality Control Test • Removal of IV Cannula • Perform 12-Lead ECG Recording
