Connected Care - Clinical Specialist
This new Connected Care Clinical Specialist role is a high-visibility group-level role responsible for providing clinical leadership and oversight for the organisation’s Connected Care initiatives across markets. Connected Care emphasizes personalised, seamless care journeys to transform the care experience for our client's customers by ensuring access to the right care, in the right place, at the right time (and at the right cost). This role requires active involvement in the development and implementation of Connected Care programs in an agile fashion.
This role is essential in ensuring clinical excellence, enhancing patient outcomes, and fostering collaborative care models. This will be achieved through innovation in healthcare delivery, patient experience and seamless integration of clinical processes, digital health and in-person care. The Clinical Specialist will contribute significantly to the development and implementation of Connected Care programs, working closely with clinical teams, technology partners, and administrative stakeholders.
Key Responsibilities:
- Cross-functional competence to drive collaborative care models delivering excellence in clinical care outcomes, patient experience and sustainability.
- Work synergistically, as a key member of an agile team, with other programme leads across markets in executing and improving the Connected Care programmes with prudent resource management.
- Provide leadership in clinical development and execution of Connected Care programmes and initiatives as aligned with organisational goals.
- Proactively work with internal and external stakeholders to ensure strategic alignment and smooth implementation of the Connected Care initiatives in all relevant areas, including regulation, risk, product, marketing and communication.
- Support smart and practical deployment of technology and digital tools to properly scale and improve the effectiveness and efficiency of the Connected Care initiatives across markets.
- Ensure all tracking and reporting for the Connected Care projects and initiatives across markets are updated and communicated in a timely fashion to relevant stakeholders.
- Ensure punctual delivery of key milestones, products and services as part of the Connected Care initiative.
- Engage actively in horizon-scanning to refine and enhance the Connected Care initiatives and projects, implementing best practices and standards wherever applicable.
- Advocate and be the champion of Connected Care for the organisation.
Requisites:
- Essential: Degree in Nursing, Medicine, or a related health field; postgraduate qualifications preferred.
- Specialisation in healthcare administration, public health or a related field will be an advantage.
- Current working knowledge of health system processes such as discharge planning, utilization management, case management, performance improvement and managed care reimbursement will be highly desirable.
- Strong understanding of clinical workflows and healthcare technology required.
- Understanding of the market variability in pre-post care, care access and community resources among different health jurisdictions, especially in Asia, will be a great advantage.
- Minimum of 7-10 years of clinical experience, with a focus on implementing and scaling digital health, connected care and/or care programmes preferred.
- Experience in developing and implementing care pathways and disease management programs. Knowledge and implementation of evidence-based practices in healthcare delivery required.
- Experience in utilization management, case management, discharge planning or other cost/quality management program
- Proven experience in managing cross-functional teams (includes technology as a stakeholder)
