POSITION SUMMARY:

The employee shall complete the training, assessment and all activities as stipulated in the Records of Training and Experience of Provisionally Registered Pharmacist (PRP) under the supervision of the appointed preceptor.

DUTIES & RESPONSIBILITIES:

1. Ensure the neatness and cleanliness of workplace.

2. Verification of the appropriateness of the prescription received.

3. Preparation of medicine for the outpatient and inpatient in accordance with the doctor prescription.

4. Data entry into the HIS based on the prescription received.

5. Clarify with the prescriber if there were doubt on the prescription.

6. Fill and label medicine as prescribed correctly.

7. Preparation of extemporaneous preparation as and when needed.

8. Dispense medicine, counsel patient (as and when required) by informing the patient on the dosage, frequency and duration of the medication.

9. Indent stock from the pharmacy store daily.

10. Ensure the recording of the control medicine dispense to patient as required by the law.

11. Manage the stock inventory at the pharmacy to ensure sufficient stock, FEFO, minimize wastage and checking the expiry date.

12. Able to work extra hours or whenever being called for duty by the in charge.

13. Participate in the staff development program and subject to yearly performance evaluation.

14. Participate in the company improvement activities.

15. Interact effectively with other members of the healthcare team.

16. Maintain the confidentiality of the company and patient.

17. Participate in Pharmacy-related activities e.g. World Pharmacy Day, Pharmacy Bulletin, Pharmacy Drug Talk etc.

QUALITY - PATIENT SAFETY & RISK MANAGEMENT.

1. To adhere to all Acts, Laws, Rules & Regulations governing the pharmacy services at all the times.

2. Report all accidents / incidents and hazard to the superior and complete the Incident Report Form for each accident / incident.

3. To perform any other duties as assigned by designated by the organization.
