Medical Record Assistant
Job Responsibilities
· Check deficiency and error from all clinical documents pertaining to medical records returned prior to filing and safe keeping according to medical records checklist, any incomplete form will be returned back to respective wards or clinics.
· Scan all returned document (inpatient and outpatient) accordingly. Ensure the quality of images is priority when scanning process is done.
· Uploaded all scanning documents into the system according to patient MRN. Ensure the process of uploading is done correctly into the correct MRN of patient.
· Ensure all medical records document are filling accordingly, the arrangement and filings of loose medical record form are correctly file into the medical record folder.
· Retrieve copy medical information from patient medical records and to answer authorised requests in support of patient care activities.
· Handling requests and collections for medical reports / insurance company lawyer and/or other. Ensure and manage the copies of all Medical Reports and Insurance Claims are uploaded in the system.
· Involved directly in the process of document archiving for internal and external document storages, also al the related process in medical records disposition.
· Ensure telephones are answered on timely manner for requests of medical records, medical report or insurances claims inquiry, as to facilitate efficient communication and section flow of daily activities.
· Ensure to notify notification of communicable diseases case on timely manner via fax and communicable disease system.
· Released new medical certificate to respective wards or clinics and checked on received medical certificate returned to MRD by wards or clinics.
· Personally adhere and uphold the Patient & Family Rights and Responsibilities (PFRR) in handling patients and their families at all time.
· Take part in all quality and safety related initiatives and contribute towards the continuous improvement of the hospital.
· Assist on Coding and Classification of Diseases.
· Perform any other duties that may be assigned by the HOD and/or management from time to time on a project-to-project basis.
Requirements
· Candidate must possess at least SPM or Diploma level in any field.
- Good command of both written & spoken in English and Bahasa Malaysia.
· 1-2 years’ experience, preferably in Medical Records Department.
· Healthy physically and mentally.
· Computer literacy with basic knowledge in Microsoft Office is required.
· Honest, trustworthy and able to maintain confidentiality.
· Good interpersonal and communication skills.
· Good team player.
Job Type: Full-time
Pay: RM1,700.00 - RM1,900.00 per month
