JOB DESCRIPTION – STATE REGISTERED NURSE The State Registered Nurse will assume the following roles and responsibilities: 1. Provider of Care As a provider of nursing care, the Registered Nurse uses the Clinical Pathway / Nursing Process to formulate and maintain individualized nursing care plans by: 1.1. Assessing Patient 1.1.1 Collects and contributes to a database (physiological, emotional, sociological, cultural, psychological and spiritual needs) from available resources (e.g. patient, family, doctors’ notes and others). 1.1.2 Identifies and documents changes in health status which interferes with patient’s Activities of Daily Living (e.g. oxygen, nutrition, elimination, activity, safety, rest, sleep and psychological well-being). 1.2. Planning for Nursing Care 1.2.1 Establishes nursing goals and patient expected outcomes based on the list of problems with patient, family and significant others. 1.2.2 Develops individualized nursing care plans based upon the problem list and plan intervention that follows established nursing protocols (e.g. Clinical Pathways). 1.2.3 Identifies criteria for evaluation of individualized care plans. 1.3. Implementing the Care Plans 1.3.1 Carries out individualized plans of care according to priority of needs and established nursing protocols. 1.3.2 Participates in the prescribed medical regime by preparing, assisting and providing follow-up care to patients undergoing diagnostic and / or therapeutic procedures. 1.3.3 Uses nursing knowledge and skills and protocols to assure an environment conducive to optimum restoration and maintenance of patient’s normal Activities of Daily Living (ADL). 1.3.4 Maintains and promotes respiratory function (e.g. 02 therapy, positioning, etc.) 1.3.5 Maintains and promotes nutritional status (e.g. Dietary regimes, supplementary therapy, I.V. infusions, etc 1.3.6 Maintains and promotes a balance of activity, rest and sleep (e.g. exercise, sensory stimuli, assist devices, etc.). 1.3.7 Maintains an environment which supports physiological functioning, comfort and relief of pain. 1.3.8 Maintains and promotes all aspects of hygiene. 1.3.9 Maintains and promotes physical safety (e.g. implementation of medical and surgical aseptic techniques etc.). 1.3.10 Maintains and promotes psychological safety through consideration of each individual’s worth and dignity and apply nursing measures in reducing common developmental and situational stress. 1.3.11 Measures basic physiological functions and report significant findings (e.g. vital signs, fluid intake and output, etc.). 1.3.12 Administers prescribed medication safely. 1.4. Intervene in situations when: 1.4.1 Basic life support systems are threatened (e.g. performing of CPR, obstructive airway manoeuvre, etc.). 1.4.2 Untoward physiological or psychological reactions are probable. 1.4.3 Changes in normal behaviour patterns have occurred. 1.4.4 Participates in established institutional emergency plan. 1.5. Evaluating Planned Care 1.5.1 Uses nursing process components for evaluation of individualized nursing care. 1.5.2 Participates with patients, families, significant others and members of nursing team in the evaluation of established patient’s goals. 1.5.3 Identifies alternative methods of meeting patient’s needs, modifies plan of care as necessary and document changes. 2. Role as a Patient Educator As a teacher of a patient who, the Registered Nurse shall: • Assess situations for need of information or support to maintain health. • Develop short-range teaching plans based upon long and short-range goals for individual patient • Implement teaching plans that are specific to the patient’s level of development and knowledge. • Support and reinforce the teaching plans of other health professionals. • Evaluate the effectiveness of patient’s learning. 3. Role as a Communicator As a communicator, the Registered Nurse shall: 3.1 Assess verbal and non-verbal communication of patients, families and significant others based upon knowledge and techniques of interpersonal communication. 3.2 Use line of authority and communication channel within the work setting appropriately. 3.3 Use communication skills as a method of data collection, nursing intervention and evaluation of care. 3.4 Document and record assessment, nursing care plans, interventions and evaluation accurately and promptly. 3.5 Establish and maintain effective communication with patient’s, families, significant others and health team members. 3.6 Communicate patient’s needs through the appropriate use of referrals. 3.7 Evaluate effectiveness of one’s own communication with patients, colleagues and others. 4. Role as a Member within the Profession of Nursing As a member within the profession of Nursing, the Registered Nurse shall: 4.1 Be responsible for her / his nursing practice. 4.2 Practice within the profession’s ethical and legal framework. 4.3 Assume responsibility for self-development and use resources for continued learning. 4.4 Consult with the Team Leader in the absence of Nurse Manager when patient’s problems are not within the scope of practice. 4.5 Encourage to participate within a structured role in research (e.g. data collection). 4.6 Work within the policies of the employing institution (THKD). 4.7 Recognize policies and nursing protocols that may impede patient care and work within the organizational framework to initiate change. 5.1 At all At all times, to maintain strict confidentiality of any information you may become aware of during the course of your duties. 4.8 Enhance professional growth and development through reading of relevant literature, as well as attendance to relevant training programmes (internal & external). 4.9 Perform other tasks / duties as assigned. JOB REQUIREMENTS: - • Possess a Diploma / Degree in Nursing only. • Minimum 3 years of working experience in nursing field. • Post Basic will be an added advantage. • Must have valid Temporary Practising Certificate (TPC) issued by Malaysia Nursing Board. • Required to work shifts. • Required to work in Kota Damansara.
